Investigation Report Template: How to Structure Scope, Timeline, Sources and Findings

How to use the outline

Draft from the underlying case record rather than filling each prompt from memory. The outline is a writing scaffold: requirements, terminology and section order should be adapted to the organization and type of investigation.

Scope

State what was examined, the relevant period and important boundaries. If the scope is still unclear, use the Investigation Scope Template first.

Chronology and supporting records

Include the material sequence needed for the reader to understand the matter. Keep source references available so important dates and statements remain traceable to the working file.

Interviews and contacts

Summarize relevant information rather than pasting full interview notes into the report. Identify unresolved conflicts when they affect the analysis.

Findings, limitations and follow-up

Separate documented information from analysis and conclusions. Preserve unavailable information, unresolved issues and remaining work rather than forcing every question into a definitive result.

Before finalizing

Use the Investigation Report Review Checklist to check chronology, source traceability, consistency and unresolved work before the report is finalized.
