Insurance Claims Investigation Documentation Templates
Organize claim-related reviews, incident records, source references, contact notes, and outstanding follow-ups. CaseWorks offers general-purpose editable digital worksheets for claims professionals, independent adjusters, consultants and lawful fact-finding work. These are not insurer-approved forms or a specialized insurance claims platform.
Pick the right tool for your claims review workflow
- Incident intake and follow-up: Incident Report & Investigation Tracker ($19) organizes reported events, connected people and contacts, sources, assigned actions and disposition.
- Chronology and outstanding questions: Timeline & Lead Tracker ($15) separates reported dates from follow-up leads, owners, next actions and outcomes.
- Record and statement sources: Evidence & Source Tracker ($19) tracks where information originated, what was reviewed and what still needs corroboration.
- Contact attempts and interview notes: Interview Notes & Contact Log ($19) structures contact records and related follow-ups.
- Connected case documentation: Investigation Case File System ($79) brings intake, timeline, leads, records, interviews and case review together.
Fictional example: conflicting incident details
A claimant provides one account of when equipment damage was discovered; a subsequent report lists a different time. Preserve both statements with source attribution, record the inconsistency as unresolved, and assign an appropriate authorized records request. Track when the additional record is reviewed and what remains uncertain. The worksheet organizes documentation; it does not decide coverage, liability, causation, fraud, or claim payment.
Critical limitations
These generic templates do not replace a claims management system, carrier policy, special investigation unit procedures, state insurance rules, mandated notices, evidence standards or legal review. They are not approved insurance forms and do not secure sensitive claims data. Keep personal, medical, financial and claim-identifying material only in approved secure systems.
Evaluate before purchasing
