An investigation referral should provide enough structured information for a receiving reviewer to understand what is being referred and what supporting information is available.
Useful referral fields
Depending on the workflow, useful fields can include referral date, referring source, case identifier, issue description, relevant people or roles, key dates, available records, prior actions and contact information appropriate to the case.
Separate reported information from verified information
Preserve what was reported without rewriting allegations or initial information as established findings.
Identify supporting records
Reference documents or other source material that accompanies the referral so the receiving reviewer can locate the original information.
Capture immediate questions
Record what the referral is asking the investigation to clarify, while allowing scope to evolve after intake and review.
