Investigation Information Request Tracker: Track Requests, Responses and Follow-Up

An investigation information request tracker manages records or information that have been requested but are not yet fully resolved. It keeps request dates, recipients, response status, partial responses and next-review dates visible so outstanding items do not disappear into email.

Once a requested record is received, index the actual record in the Investigation Document Log.

Information request tracker fields

Field Purpose
Request ID Stable reference for the request
Information requested Specific record, clarification or information sought
Recipient/source Person, organization or authorized source asked
Request date/method When and how the request was made
Status Sent, acknowledged, partial, complete, unavailable or other workflow status
Response date When a response or partial response arrived
Outstanding item What remains missing after any response
Next review date When the request should be checked again
Owner/next action Who is responsible and what should happen next

Track partial responses separately

A response does not always complete a request. Record what was received and what remains outstanding rather than marking the entire request complete. This makes incomplete production or unanswered portions visible during review.

Use request aging and review dates

Request date, last response and next-review date make it easier to identify items that have been waiting unusually long. The tracker should show why an item remains open and when it will be reviewed again.

Information request tracker vs. document log

The request tracker manages work that is still being pursued. The document log inventories records that have been received or identified. When a response produces a record, close or update the request as appropriate and create the corresponding document-log entry.

Connect responses to the case

When information arrives, route material dates, new contacts, sources, conflicts or resulting actions to the appropriate chronology, interview, source, lead or action record. Do not let the request tracker become the only place where substantive information is recorded.

Frequently asked questions

When is an information request complete?
When the requested information has been resolved according to the applicable workflow—not merely because some response was received.

How should partial responses be tracked?
Record what arrived, what remains outstanding and the next review or follow-up action.

Should received documents stay only in the request tracker?
No. Index the actual records in the appropriate document/source system so they can be referenced throughout the case.
