An investigation decision log can preserve significant case-management decisions separately from day-to-day notes. It helps later reviewers understand what was decided, when and what information was considered.
Useful decision-log fields
Consider decision ID, date, issue, decision or direction, supporting source references, known limitations, person or role responsible and resulting actions.
Reference supporting information
Link significant decisions to the records, interviews, chronology entries or other information considered rather than relying on an unexplained conclusion.
Preserve uncertainty
If a decision is provisional or based on incomplete information, record that limitation and any planned review point.
Track resulting actions
If a decision creates follow-up, move those steps into the action tracker while retaining the decision record.
