Investigation Contact Log Template: Track Contacts, Attempts and Follow-Up

An investigation contact log provides one place to see who was contacted, when contact was attempted, what happened and what follow-up remains.

What to track

Useful fields can include person or organization, role, contact method, date and time, attempt number, status, brief outcome and next action.

Separate contact history from interview content

The contact log should show the communication history. Detailed interview notes or statements belong in the appropriate interview record.

Record unsuccessful attempts

Unanswered calls, returned messages and other attempts can matter to workflow. Keeping them visible prevents duplicate effort and missed follow-up.

Connect follow-up to actions

If a contact produces a new task, lead or record request, move that work into the appropriate tracker rather than leaving it buried in a contact note.
